Diagnosing Discrepancies in Conversion Rate Tracking Issues

Help me diagnose why this tracking isn't working correctly. There is no way our conversion rate from sign up to doing a mission is 2% (we have a totally different number in our database if we query it). Where do I go to understand why something isnt connecting right?
